Medicare Advantage vs. Medicare Supplement Insurance (Medigap): What’s the difference?

Once you have Medicare Part A and Part B (Original Medicare), you still pay a share of your costs — deductibles, copays and coinsurance, with no yearly limit. There are two main ways to help manage those costs: a Medicare Advantage plan, or a Medicare Supplement (Medigap) policy. They work very differently, and you can’t have both at the same time.

Option 1

Medicare Advantage (Part C)

An all-in-one alternative to Original Medicare, offered by private companies that are approved by Medicare. You still have Medicare, and you keep paying your Part B premium.

  • Includes your Part A & Part B hospital and medical coverage
  • Most plans include Part D prescription drug coverage
  • Often adds extras like dental, vision, hearing & over-the-counter allowances*
  • Uses a provider network (HMO or PPO) and set copays
  • Has a yearly limit on what you pay out of pocket for covered services
Option 2

Medicare Supplement (Medigap)

A policy sold by private insurance companies that works alongside Original Medicare and helps pay some or all of your share of covered costs.

  • Helps with deductibles, copays & coinsurance, depending on the plan letter
  • See any doctor or hospital in the U.S. that accepts Medicare
  • Does not include prescription drug coverage — you add a separate Part D plan
  • Does not usually include dental, vision or hearing
  • You pay a separate monthly premium on top of your Part B premium

After that, insurers in many cases can ask health questions and may charge more or decline.
Can I have both? No. If you have a Medicare Advantage plan, you generally can’t use a Medigap policy at the same time.

Medicare Advantage may fit if you…

want one plan that bundles medical, drug and extras, prefer predictable copays and low or $0 premiums, and are comfortable using a network of doctors.

Medigap may fit if you…

want the freedom to see any Medicare provider nationwide, want to limit your cost-sharing, and don’t mind paying a separate premium and adding drug coverage.

Not sure which is right for you?

We’ll walk through both options in plain English — your doctors, your medications, and your budget — at no cost to you.

Living with a health condition?

High blood pressure or diabetes won’t keep you from enrolling.

With a Medicare Advantage plan, you can’t be turned down or charged more because of a pre-existing condition like high blood pressure or diabetes, as long as you enroll during a valid enrollment period. What matters is choosing a plan that fits how you use care. We’ll help you check:

  • Your doctors and specialists are in the plan’s network
  • Your prescriptions — including insulin — are covered, and what they cost
  • Copays for primary care, specialists, labs and supplies
  • The plan’s yearly out-of-pocket maximum
  • Extra benefits that may help, like OTC allowances and wellness rewards*

Medicare drug plans cap covered insulin at $35 per month. Medicare Advantage eligibility requires Medicare Part A and Part B and residence in the plan’s service area. Medigap works differently: outside your Open Enrollment or other guaranteed-issue rights, insurers may consider health history.

Questions

Medicare answers in plain English

What do I need to join a Medicare Advantage plan?

You must have Medicare Part A and Part B, continue to pay your Part B premium, and live in the plan’s service area.

When can I enroll?

The Medicare Annual Enrollment Period runs October 15 – December 7, with coverage starting January 1. You may also enroll when you first become eligible, during the Medicare Advantage Open Enrollment Period (January 1 – March 31, if you already have a Medicare Advantage plan), or during a Special Enrollment Period if you qualify.

Will I be denied because of high blood pressure, diabetes or another condition?

No. Medicare Advantage plans can’t deny you or charge you more because of a pre-existing health condition when you enroll during a valid enrollment period. Medigap works differently, so ask us about the difference.

Can I keep my doctors?

It depends on the plan’s network. Tell us your doctors and hospitals and we’ll check before you enroll. PPO plans also let you see providers outside the network, usually at a higher cost.

Are my prescriptions covered?

Most Medicare Advantage plans include drug coverage, and each plan has its own list of covered drugs (formulary). Share your medication list and we’ll compare costs.

What does it cost to work with an agent?

There is no cost to you for agent help. Plan premiums are the same whether you enroll through an agent or directly.

Can I have Medicare Advantage and Medigap together?

No. Medigap is designed to work with Original Medicare. If you have a Medicare Advantage plan, you generally can’t use a Medigap policy at the same time.
